How do I know if I am eligible for this program?
All patients with a valid prescription for FLOMAX are eligible for the FLOMAX Door-to-Door program.
How do I submit a prescription?
Prescriptions and enrollment forms can be mailed to: FLOMAX Door-to-Door c/o Eagle Pharmacy PO Box 90937 Lakeland, FL 33804.
Please be sure your date of birth is included on your prescription. If you do not have a hard copy prescription to mail, you can ask your physician to submit one to Eagle Pharmacy via:

In accordance with Board of Pharmacy Regulations, only a physician may submit a prescription through fax, phone, or ePrescribe. Patients may mail original, hard copy prescriptions to Eagle Pharmacy. Eagle Pharmacy will fill medications upon receipt unless otherwise noted on the prescription to hold/profile by the patient or physician.
What happens once I send in the enrollment form and my prescription? How do I check the status of my order?
Once we receive your completed enrollment form and prescription information, we will process your order and your medication will be shipped directly to your home within 5-9 days as long as no order processing issues arise. You can check the status of your order at any time by calling 1-855-799-6835 or by emailing BI@eaglepharmacy.com (email is monitored Monday through Friday, 8:00 AM–5:00 PM ET, excluding holidays).
Can I use my commercial insurance coverage if I enroll in the FLOMAX Door-to-Door program?
No. All Door-to-Door prescription transactions will be by credit card only. Everyone pays the same cash price of $35 for a one-month supply or $90 for a three-month supply of FLOMAX, with no insurance required.
What if I have Medicare Part D or Medicaid insurance? Can I still enroll in this program?
Yes. If you have Medicare Part D, Medicaid, or a similar state or federally funded medical assistance program, you will pay a cash price of $35 for a one-month supply, or $90 for a three-month supply of FLOMAX. All Medicare Part D orders are processed without the use of insurance and cannot be applied to Part D true out-of-pocket (TrOOP) costs. State and local taxes may apply.
Am I eligible if I don’t have any insurance? How much will my FLOMAX cost?
Patients have a choice of two payment structures: 1) Patients can enroll for a one-month supply at $35 for 30 capsules of FLOMAX, or 2) Patients can enroll for a three-month supply at $90 for 90 capsules of FLOMAX. All patients, upon initial enrollment, will receive an additional free one-month supply (30 capsules) of FLOMAX. Enrollment includes the fee payment for the initial prescription. This will likely be a significant savings over what you would pay for brand-name FLOMAX or a generic at a retail pharmacy.
How do I order a prescription refill?
You can refill your prescriptions by calling us at 1-855-799-6835. Customer Support representatives are available Monday through Friday 8:00 AM – 10:00 PM ET, excluding holidays. Eagle Pharmacy’s hours of operation are Monday through Friday, 8:00 AM–5:00 PM ET, excluding holidays.
You can also elect to refill your medication through the Automated Refill program. Each month, your medication will automatically be shipped directly to your home and the credit card on file will be charged prior to shipping. Patients are responsible for all shipments and co-pays while enrolled in this optional service. You may opt in or out at any time by speaking with one of our Customer Support representatives at 1-855-799-6835.
